GDP Survey -Scottish Dental Needs Assessment Programme (SDNAP)

The Scottish Dental Needs Assessment Programme (SDNAP) is investigating the provision of dental services to adults of age 45 and over (45-64 and 65+).

We have put together this short questionnaire to gather information on the provision of dental care to adults of age 45 and over by general dental practitioners (GDP) and I would be grateful if you could complete this as soon as possible.

There is a free text box at the end of the questionnaire if you would like to add any additional information.
